Support for health professionals

Referring someone to Support at Home

GPs, hospital discharge teams, allied-health professionals, care coordinators and other health professionals can contact us to discuss a person's support needs.

We can discuss:

  • Current support arrangements
  • Existing Support at Home approval
  • Required services
  • Community nursing needs
  • Post-hospital support
  • Family and carer circumstances
  • The appropriate referral pathway

Working alongside the healthcare team

Support at Home is often one part of a person's broader care arrangement.

Where appropriate and authorised, we work collaboratively with the person's GP, allied-health professionals, family, carers and other relevant professionals.

Our aim is to provide reliable support while respecting the person's privacy, choices and care arrangements.
