Professional nursing care delivered at home

SA Community Support provides community nursing services for people who require nursing care in their home or community.

Our nursing team supports people with ongoing health needs, clinical care requirements, post-hospital recovery and changes in health that require assessment, monitoring or nursing intervention.

Supporting people after hospital

Hospital discharge does not end at the front door

The transition from hospital to home can involve medication changes, wound care, mobility concerns, new equipment, altered routines and increased support needs.

Our community nursing team can provide appropriate nursing and support services after discharge, according to the person's needs and arrangements.

Where appropriate and authorised, we can work with the person's GP and other healthcare professionals to support continuity of care.
